Designations Eligibility Application


United Way of Calvert County provides an annual application for 501(c)3 organizations that address an identified community or health and human service need in Calvert County to receive designations from the United Way campaign.

Designations give organizations that serve an important need in the community an avenue for receiving gifts from donors. It also affords them the ability to partner with United Way of Calvert County to improve their visibility. United Way of Calvert County’s annual Designation Eligibility Application process provides a high level of accountability to our donors.

In order to equitably recoup administrative and fundraising costs, organizations receiving designations will be assessed an administrative fee of 5%. Payments are made based on actual cash received. Historically, uncollectible amounts on designated pledges have been approximately 5%.

Required Grant Attachments
  • Application (completed and signed)
  • 501(c)3 Determination Letter
  • Current letter from the Secretary of State certifying your registration status AND State Department of Assessment and Taxation (SDAT) free online certificate of status. http://sdatcert1.resiusa.org/certificate_net/
  • Board approved budget for current fiscal year
  • Complete and signed copy of the IRS Form 990 for a period ended not more than 18 months prior to January 2013
  • Most current financial statement as determined below:
    • If total revenues are $250,000 or greater, provide a copy of audited financial statements by an independent certified public accountant in accordance with Generally Accepted Auditing Standards (GAAS).
    • If total revenues are over $100,000 but less than $250,000, provide a review of financial statements by an independent certified public accountant in accordance with GAAS.
    • If total revenues are less than $100,000, provide a board signed profit and loss statement and certify use of appropriate accounting and record keeping procedures or generally accepted accounting principals (GAAP)
