Designation Eligibility Application - 2020

United Way of Calvert County (UWCC) provides the opportunity for 501(c)3 organizations addressing an identified community or health and human service need in Calvert County to apply for eligibility to receive designations from the United Way campaign.

Applicants must complete a Designation Eligibility Application & Agreement online and submit accompanying required documents on an annual basis to qualify for designations from the UWCC campaign. Decisions are recommended by the UWCC Community Impact Committee and approved by the Board of Directors based on the organization’s ability to meet the designations eligibility criteria, submit a complete application, and show financial stability.

  • Application Available: Friday, June 5, 2020
  • Application Deadline: Friday, July 10, 2020 2:30 pm

Applicants will be notified of the UWCC Board of Directors' decision after July 24, 2020.

Incomplete applications - or any applications received after Friday, July 10, 2020 will not be eligible

for consideration in UWCC's 2020-21 Campaign.


Required Documents

 

The following documents are necessary to complete the 2020 Designation Eligibility Application

 

It is recommended to have the following documents available in Excel or PDF format to upload into the application:

  • 501(c)(3) Determination Letter

  • Secretary of State Certification of Charitable Status

  • State Department of Assessment & Taxation Certificate (SDAT)

  • 990 Tax Form - completed and signed. Dated no later than 18 months prior to January 2020

  • Organization’s Financial Statement for current Fiscal Year (2020)

  • Patriot Act Compliance Form (download form, complete, and upload completed form)

  • Budget Form from the upcoming fiscal year (download budget format, complete, and upload completed form)
